    Consistency with the provider is important from an APP or a physician standpoint, and then consistency with your infusion nurse and your nurse that may be rooming you, because when you see these patients over and over again, you can notice that they are more winded when they’re walking into the room.

    Taking their pulse [oximetry] is extremely important. That’s something that we do at every single visit, because they may not want to report it, but you can see a trend: they’re usually 98 or 100 and all of a sudden, they’re 95 and that’s something that you can trend out, typically in the electronical medical records.

    And then, one thing that I love to do is ask the person who brings them or is with them, if somebody comes with them, you shoot them eyes if you think something’s off. And a lot of times, they will be behind you, saying, “Oh yes, they are way short of breath,” and they’re noticing changes.

    So pull in all your resources that you really can to try and help with that. Because a lot of them aren’t going to say they’re short of breath, or they’ll be like, “Yes, I’m short of breath when I walk from the car.” And you’re like, “OK, but when you sit, how long does it take to recover? Is it something that’s taking longer?” And those are tips that you can really try and suss out for the patient what exactly is going on.

  • FDA approves Ozempic to reduce risks from chronic kidney disease in diabetes patients

    Ozempic, the blockbuster GLP-1 drug that was originally approved to treat type 2 diabetes, has now also been approved by the US Food and Drug Administration to reduce certain risks associated with chronic kidney disease, drugmaker Novo Nordisk said Tuesday.

    A Phase 3 clinical trial found that injections of the semaglutide medication once weekly reduced the risk of kidney disease worsening by 24% in people who also had diabetes. It also reduced the risk of kidney failure and cut the risk of death from heart disease by about 5% in adults with both type 2 diabetes and chronic kidney disease, the company said in a news release.

    Chronic kidney disease is a common complication for people living with type 2 diabetes. Diabetes is the leading cause of kidney failure, which is one of the leading causes of death in the US and worldwide; about a third of adults with diabetes also have chronic kidney disease, according to the US Centers for Disease Control and Prevention

    The clinical trial tested the efficacy of Ozempic when added to the standard of care, not as a replacement for other treatments for disease management such as medicines to manage blood pressure, including ACE inhibitors and ARBs. About 3,500 adults across 28 countries were enrolled in the trial, which started in 2019 and followed participants for an average of about 3½ years.

    The new indications, approved Tuesday by the FDA, make Ozempic the most broadly indicated drug in its class, with the largest set of specific medical conditions that it may be prescribed to treat.

    Novo Nordisk is conducting a different study to understand how and why semgalutide seems to reduce the progression of kidney disease, said Stephen Gough, global chief medical officer and senior vice president at Novo Nordisk. But even though scientists don’t fully understand the mechanisms behind them, the positive effects are strong: The clinical study was stopped early after reaching positive endpoints sooner than expected, and no new safety risks were identified in this trial, he said.

    “This really means something to the patients,” Gough told CNN. “The impact on not just kidney disease but also cardiovascular disease and on all-cause death.”

    Ozempic and other semaglutide injections such as Wegovy have been in short supply for years due to their rapid popularity. The FDA now lists Ozempic as “available,” but there is still high demand.

    “This is a medicine that’s already available, both in diabetes clinics and now in renal clinics, so I would hope the uptake would start pretty quickly,” Gough said, adding that he hopes the confidence that comes with the data behind this approval will aid clinicians who are making daily decisions about how and when to use different treatments.

    “It does help clinicians with their decision-making process, and it helps them focus those medicines to the patients who will benefit the most,” he said.

  • Akero drug reverses liver scarring in study of severe MASH patients


    Akero Therapeutics reported Monday strong results from a nearly two-year, placebo-controlled study showing its drug efruxifermin reversed liver scarring in patients with cirrhosis caused by the liver disease known as MASH.

    After 96 weeks, 39% of patients offered a 50-mg dose of efruxifermin showed a clinically meaningful reduction in liver fibrosis, or scarring, without other symptoms of MASH getting worse, compared to 15% of participants randomized to a placebo. 

    Patients with cirrhosis due to MASH have a severe form of the disease with few treatment options outside of a liver transplant. Once MASH progresses to cirrhosis, median survival is about five years, three times shorter than for patients with moderate, or mid-stage, MASH.

  • A doctor accused of sexual abuse by more than 200 former patients is indicted on rape charges


    A grand jury in Massachusetts has indicted a doctor who is accused of sexually assaulting more than 200 former patients over a period of more than a decade.

    The patients sued Dr. Derrick Todd, who has been accused of performing unnecessary pelvic floor therapy, breast examinations, testicular examinations and other unwarranted procedures on patients dating back to 2010. The Middlesex District Attorney’s Office said in a statement Thursday that Todd was indicted earlier that day for “two counts of rape alleging the sexual assault of two women.”

    Todd pleaded not guilty during an arraignment on Friday. The district attorney’s office said the two alleged assaults took place in December 2022 and June 2023, and that additional investigations into Todd’s conduct are ongoing.

    The criminal case against Todd will likely grow, said William Thompson, an attorney with Boston firm Lubin & Meyer, which represents more than 180 of the accusers.

    “It’s just the beginning of the criminal case against Dr. Todd, but it does help validate the civil claims that Lubin & Meyer is pursuing on behalf of so many of his former patients,” Thompson said. “Fundamentally, it’s about a doctor abusing his position. And taking advantage of patients who put their trust in him for his own personal sexual gratification.”

    Thompson added that the civil case against Todd is currently in the process of discovery. The Associated Press left a telephone message with Anthony Abeln, an attorney who has represented Todd in the civil case, and with Ingrid Martin, a criminal lawyer who has represented Todd.

    Todd is a former rheumatologist at Brigham and Women’s Hospital in Boston. The lawsuit against him accused several other defendants, including the hospital, of knowing about the abuse and failing to halt it.

    Brigham and Women’s received anonymous complaints about Todd in April 2023 and launched its own investigation. The hospital fired him in July. Todd then reached a voluntary agreement with the Board of Registration in Medicine to stop practicing medicine anywhere in the United States.

    Todd’s former places of work have distanced themselves from the doctor since the allegations emerged. His accusers range from teenagers to women in their 60s.

    Paula Bliss, an attorney for victims and partner with Justice Law Collaborative, a Massachusetts legal advocacy group, said Friday that the indictment was “a resounding moment of validation for the patients he sexually assaulted and the brave survivors who came forward with the truth of this so-called medical professional’s deplorable conduct.”

    Upland police and emergency services told CBS News Philadelphia Thursday night that they are responding to a water main break and electrical fire inside the Crozer-Chester Medical Center in Upland, Pennsylvania.

    Upland Police Chief Michael Irey said about 24 patients from the hospital are being evacuated.

    Crozer-Chester CEO Tony Esposito said in an emailed statement, “We experienced a small flood that resulted in a fire, but there is no longer an active fire. Critical care patients were evacuated from the facility to safe areas and we have been working with the county ensure their safety. No injuries have been reported.”    

    A spokesperson for PECO said crews were on the scene and part of the building lost power.
